POLK CITY -- Mr. Tommy W. Chism of Polk City died of heart failure Sunday (Jan. 1, 2006) at Kindred Hospital in Tampa. He was 58.
Born in Tryonza, Ark., on March 28, 1947, he came to Haines City from there 48 years ago. He was vice president of Deck Systems Inc. in Altamonte Springs. He was a member of the Haines City Moose Lodge.
Mr. Chism is survived by his wife, Angela Chism; sons, Tommy Chism Jr., Scott Chism and Adam Chism, all of Polk City; daughter, Amanda Chism, Polk City; brothers, James Chism, Haines City, and Billy Chism, Lake City; sisters, Ethel Ford, Polk City, and Shirley Qualls, Bartow; and six grandchildren.
Visitation will be from 5 to 7 p.m. Friday at Oak Ridge Funeral Care, Haines City. Services will be at 2 p.m. Saturday at the funeral home chapel. Burial will be in Forest Hill Cemetery in Haines City.
